Forms You Need for Radius
Okay, we know this is a bunch of hassle, and we wish we didn't have to do it. But, our insurance company says we have to ask you to fill out a pile of papers. All of these are for your protection and ours, so you can be involved in the fun things and service projects we're doing.
SOOOOO, here's the stuff...
Click here to download the required forms for participating in Radius off-campus events over the summer. This form must be signed by both parents, or the custodial parent in a single-parent family, and notarized. The completed form must be turned in to the church office before the student participates in any summer, off-campus activities.
Click here to download a permission slip for specific and one-time events. This form needs to be filled out and turned in before participating in the activity.
